Guidelines for Management of Persons with Hypertension and High Cholesterol Using Patient-Centered Team Based Care

Guidelines for management of persons with hypertension and high cholesterol educational video was developed for primary care clinicians and their support staff to demonstrate how team based care can be utilized in improving  two prevalent chronic conditions; hypertension and high cholesterol. This video presents the model and guidelines for the identification, treatment and management for the two chronic diseases. These guidelines build upon the existing and emerging work of many partners in Wisconsin and the nation to foster health care transformation that advances patient-centered team-based care and move toward value-based care, improved patient health and safety, and improved health of the population.

CE Credit:  .75 contact hours for completing the Online Self-Study are available to participants who view the entire presentation and complete the online survey/evaluation before 12/31/2021.

Diabetes and Prediabetes Care Involving DCES/CDE in Disease Management

This presentation will promote the team approach to Prediabetes and Diabetes management, with attention to the important role of the Diabetes Care and Education Specialist/Certified Diabetes Educator, and referrals to Diabetes Self-Management Education Services (DSMES). Will also discuss the National Diabetes Prevention Program – a partnership of public and private organizations working to prevent or delay type 2 diabetes. Will address medications useful in disease management and strategies for maintaining a team management model during the COVID-19 Pandemic.  (1 hour 54 minutes duration)

CE Credit:  2.25 contact hours (including .75 pharmacotherapeutic content) are available to participants who view the entire presentation and complete the online survey/evaluation before 12/31/2021.
